Chronic Disease Management

Carea Community Health Centre (CCHC) operates multiple  services and programs operating across 3 sites. Everyone works together to address the diverse health needs of individuals and families living within the Durham Region.

Chronic diseases, such as Diabetes and Hepatitis C viral infection are growing in prevalence. Clients living with a chronic disease require a wide range of services and supports that evolve and vary over the course of their lives and conditions. In addition, with an increase in the population of frail older adults, improving health care for these seniors is a top priority of the Central East LHIN and Ontario’s Ministry of Health and Long Term Care.

The Programs and Initiatives that fall under the umbrella of Chronic Diseases Management are focused on working with individuals, families, and communities to help people maintain as much health and independence as possible.  This is achieved through prevention, early detection, and management of chronic health conditions, supporting patients in self managing their chronic disease and helping them to coordinate and access necessary care throughout their disease progression. The Chronic Disease Management Program is focused on providing timely, high quality services in the most appropriate setting and with seamless access to diagnostics, assessments, interventions and education.
